Requesting Copies of Your Medical Record
Copy Fees may apply - Please contact our copy service at Ext 1538 for information about copy fees.
Turnaround Time - Our quality standard is to process your request for copies in 10 to 14 days. (Some restrictions may apply).

Authorization Form - A signed, completed authorization form by you or your legal guardian is required in order to process your request for medical records. You may FAX your completed form to (920) 623-1581. You may also mail it to:

Requesting Copies of a Birth Certificate - Columbus Community Hospital does not maintain copies of Birth Certificates. Copies may be requested from the Columbia County Register of Deeds in Portage, Wisconsin. For more information about this, go to: http://www.co.columbia.wi.us or call (608) 742-9677.
