OUR MISSION:
The mission of Columbus Community Hospital is to identify and meet the healthcare and wellness needs of the people and the communities we serve.
Payment Options

For the convenience of our patients Columbus Community Hospital accepts the following:

·         Cash

·         Check or Money Order

·         Visa, Master and Discover Card

·         Health Care Credit Card

If you wish to pay online by credit card please visit our secure Bill Pay Service section.

 

You may qualify for services at a reduced cost or no charge through Columbus Community Hospital’s Community Care Grant Program or you may be eligible for assistance through a government program.

 

Columbus Community Hospital’s Community Care Grant Program is intended to help individuals and families who would experience extreme hardship while meeting their full financial obligation to the hospital. If you are concerned that you may not be able to pay for your bill in full, please contact our Financial Counselor.

Our Community Care Grant Application is available in PDF format.  If you can not print the application, call our Financial Counselor to request one be mailed to you.
Complete the attached form and FAX it to (920) 623-1578, or mail it to Columbus Community Hospital, ATTN: Patient Financial Counselor, 1515 Park Avenue, Columbus, WI 53925.
