Our swing bed program provides an extended care opportunity for individuals who are continuing to recover, but no longer need an acute hospital setting. Specifically, it is for patients who no longer need the acute level of care following an illness, stroke, injury or surgery. Yet, they are not quite physically able to return home and need a few more days to heal and regain their strength.
Our expert care team works with qualified patients to create unique, multi-disciplinary treatment plans.
We’ll help you improve your strength and gain the independence you desire before returning to your home and family.
Care teams and treatment plans may involve any of the following:
- Physical Therapy
- Occupational Therapy
- Speech Therapy
- Nutrition Services
- Diabetes Education
- Pharmacy
- Recreational Activities
- Pulmonology/Respiratory Therapy
- RN Case Manager/Social Work
So whether you’ve had surgery at Columbus Community Hospital or somewhere else, you can recover right here and we’ll help you get back to doing the things you enjoy.
