CCH's Community Care Grant Program is intended to assist individuals or families who would experience extreme hardship while meeting their full finanical obligation to the hospital. Community Care is not an entitlement program.
Q: How do I qualify for Community Care?
A: The first step is to submit an application. It is available in PDF format. Click on Community Care Application, print the application, complete it, and return it with supporting documentation to:

If you wish to receive an application in the mail, please call (920) 623-2200 or (800) 549-7511 and ask to speak to a Patient Financial Counselor.
Q: What supporting documentation do I need to include with my application?
A: Required documents include:
- copies of the three (3) most recent paystubs for all adult family members
- copy of most recent federal tax return
- copy of unemployment or pension compensation
Patient Financial Counselors may require other verification of income or assets when deemed necessary.
Q: How long does it take to know if I qualify?
A: Applicants will be notified in writing of approval or denial within 30 days of receipt of a complete Community Care Application. An application is considered complete when all requested supporting documentation has been received.
